Japan Association of Public Health Achievement Award Application Guidelines
The Japanese Association of Public Health is accepting applications for the award to recognize individuals who have made outstanding achievements through sincere public health activities over many years and contributed to the development of the organization. The award is as follows:
Please refer to the following application guidelines and recommend suitable candidates.
1. Recipients of the Award
The recipient must be a member of the Japan Association of Public Health (General Incorporated Association) and meet all of the following conditions. However, current officers are not eligible for the award.

(1) Anyone aged XNUMX or older
(2) A person who has been a member of the corporation for XNUMX years or more
(3) A person with XNUMX years or more experience as a council member or delegate
(4) A person who has made outstanding achievements in research or practical activities in the field of public health and contributed to the development of the corporation, or who has made great efforts in the development of the corporation as a president of a society, a chairman of the board of directors, or the editor-in-chief of the Japanese Journal of Public Health.
2. Selection of the Special Achievement Award
The selection will be made by the Distinguished Service Award Selection Committee.
3. How to apply and notification of results
① Application period: April 2026, 4 to August 1, 2026
② How to apply: Please fill out the application form and send it to [phgakkai@jpha.or.jp]. Applications can be submitted either by self-nomination, recommendation by others, or both. In the case of recommendation by others, the person concerned must understand that they will be subject to the award selection process.
③ Notification of Selection: The selection results will be notified to the winners by early October 2026.
4. Awards
The award will be presented at the General Meeting of the Japanese Association of Public Health.
5. Others
Personal information entered in the application documents will be used solely for the purposes of judging and presenting the Merit Award.
